Configure an Ad Hoc form
  1. From the Navigation bar under Site, select Manage Navigation.
  2. Scroll to the section in which you want to add the node.
  3. Click the Insert Node link.
  4. Click the Select custom node to add radio button.
  5. In the Node Type drop-down, select Assessor Initiated Assessment.
  6. Complete the fields on the page as appropriate.
  7. Scroll to the bottom of the screen and click Next.
  8. Complete the fields on the page as appropriate.
  9. Click Save.
