eForm elements

Basic elements

Basic elements are the fundamental parts of an eForm.

Element type

Description

Section

Used to visually breakup the display of the eForm and for creating forms that multiple employees will be entering information. Section permissions control who can fill out the fields within a section.

Static Text

Multiple lines of text that allow for some basic formatting.

Text Input

Label and input field for text.

Date Field

Label and input field for a date.

Selection

Label and drop-down, check boxes or radio button input field.

Horizontal Rule

View video

Line across the form to provide a visual separation.

Advanced elements

Advanced elements are more complex elements that control the display or special groups of fields.

Element type

Description

Banner

View video

Used to display the form title and an optional logo. Must be the top element on a form and each eForm can only have one Banner.

Footer

View video

Used to display custom text on an eForm. Each eForm can only have one footer element.

Full Name

View video

Presents First, Middle and Last name fields horizontally. Salutation and Suffix are optional.

Electronic Signature

Used to collect an electronic signature on the eForm.

Acknowledgment

View video

Used to build a link to a document or page that must be opened (clicked) before the form can be completed. Uploaded documents can be browsed and selected later. The uploaded documents cannot be deleted unless they are removed from Eprise user interface.

Address Block - US

View video

Presents a standard United States address block including Address lines 1 and 2, City, State and Zip code.

Address Block - CA

Presents a standard Canadian address block including Address lines 1 and 2, City, Province and Postal code.
